How to Participate


NorthPoint Health and Wellness Center
The Healthy Heart program at the CCOE can help you make important lifestyle changes for your cardiovascular health. Our nurse coordinator will help you every step of the way, from keeping track of appointments to helping you stay on track of a fitness program. We offer a free heart risk assessment. Call to make an appointment for your free initial heart risk assessment.
