NorthPoint Health and Wellness Center
The purpose of the National Community Center of Excellence (CCOE) in Women’s Health at NorthPoint Health & Wellness Center is to improve health and wellness for all women and remove barriers to health care access. Their goal is to integrate health and human services and provide a one-stop center for comprehensive services.

The CCOE offers educational health programs and resources designed especially for women to help them make positive choices regarding their health and well-being.

NorthPoint offers a full range of medical, dental and mental health services. At the CCOE, they specialize in women’s heart health and prenatal care.

Minnesota Department of Health
The Minnesota Department of Health aims to protect, maintain, and improve the health of all Minnesotans.  (Partners are Cancer Control Section, SAGE, and SAGEplus)  The Minnesota Department of Health will provide data management to the project.  The Minnesota Women’s Healthy Heart intervention is based on an Minnesota Department of Health administered program called SAGEplus.  To learn more about this program, please visit http://www.health.state.mn.us/divs/hpcd/ccs/sageplus/sageplus.htm

Fairview Range Regional Health Services Mesaba Clinic
The entities that comprise Fairview's Range Partners continually prepare for advances in technology and challenges in health care delivery. Fairview’s long-standing tradition of providing high quality health care and our commitment to the pursuit of continuous quality improvement continues.

Fairview combines the area's finest medical services to provide the most comprehensive health care system on the Iron Range and northeast Minnesota. This locally integrated health care system is comprised of three entities:

Fairview University Medical Center-Mesabi - main facility, located in Hibbing, providing advanced, high technology, inpatient and outpatient care

Fairview Mesaba Clinics - community-based clinics throughout the region, with sites in Hibbing, Chisholm, Mountain Iron and Nashwauk, provide the physician partnership component and community/regional access to primary and specialty care

Fairview HealthLine - which provides the community-based home health care and hospice services that support and complement both UMCM & Mesaba Clinics in the overall continuum of patient care. Medical Supply stores in Hibbing, Virginia and Int'l Falls provide delivery throughout all of Northeast Minnesota; and HomeCare services offered across the entire Range Region with main office located in Hibbing and also serving the Vermillion area.

NortheastArea Health Education Center
Northeast Minnesota AHEC promotes rural health educational opportunities and addresses health workforce challenges unique to the ten-county Northeast region.  Northeast Minnesota AHEC will work closely with the Hibbing Mesaba Clinic to expand cardiovascular screening services to rural and Native American women in the Hibbing and Nett Lake Reservation areas.
